In the changed time when teleconsultation has attained much more importance than ever before, IADVL Academy endeavoured to formulate a consent form for teleconsultation and a teledermatology case record form. Our members may consider using the consent form, even though the consent is implied if a patient initiates teleconsultation as per recent National Medical Commission guidelines. You may also adapt the teledermatology case record form as per requirement.
